Based on our review of the project manual and bid documents for Park Development at Gutchess Lumber Park - the documents will be in conformance with the work described in Attachment B-3 Capital Based Budget of the Master Contract and will have no adverse impact upon historic resources. The final Project Manual and Drawings must be stamped by the preparer to verify a license to practice in the State of New York. Please mail or email the final stamped plans to Nicole Freeman at: Nicole Freeman Grants Administrator, Central Region New York State Parks, Recreation & Historic Preservation Clark Reservation State Park 6105 East Seneca Turnpike Jamesville, NY 13078 Nicole.Freeman@parks.ny.gov The Project Manual and Drawings are part of the above referenced Master Contract, and no changes may be made to the documents without the express written consent of this office. A complete set of the documents shall be kept on the project site at all times. All addendums and change orders must be submitted to State Parks prior to approval. Please remember, the Town of Cortlandville will need to contact NYS certified Minority and Women- Owned Business Enterprises (M/WBE) to inform them of the bid opportunity. Contact can be through certified mail, email or fax. Town of Cortlandville must keep copies of all outreach efforts and document the outreach on the enclosed Contractor's Solicitation Log. Town of Cortlandville can search the NYS M/WBE Directory for businesses to contact at: https://ny.newnycontracts.com/ Once the bids are opened and the lowest bidder has provided a completed Utilization Plan, Town of Cortlandville will notify OPRHP to open an account in the NYS Contracting System. Once the account is opened, Town of Cortlandville will be given 10-days to upload the Utilization Plan for review. The NYS Affirmative Action Office will review the Utilization Plan, and all documentation of the outreach efforts to achieve the 30% participation goals assigned to the grant award, and either approve or deny. If the Utilization Plan is approved, Town of Cortlandville will be informed to contact the lowest bidder and obtain the remaining required documents to execute a construction contract. If the Utilization Plan is denied, Town of Cortlandville will be instructed on how to proceed -either seek more participation or submit a waiver request. Review of the Utilization Plan is completed within 20-days. For your information, the additional required documents to execute a construction contract are: 1. Vendor Responsibility Questionnaire: Construction - For-Profit (only for contracts over $100,000) 2. Equal Employment Opportunity Policy Statement 3. Proof of Worker's Compensation and Disability Benefits Insurance coverage. The following are the only acceptable means of proof (please note that ACORD forms are NOT acceptable proof of coverage). The contractor will list Town of Cortlandville as the Entity Requesting Proof. A copy of the insurance documents will be provided to State Parks. Disability Benefits: DB-120.1: Certificate of Disability Benefits Insurance; or DB-155: Certificate of Disability Benefits Self-Insurance. Workers' Compensation: C-105.2(9-07): Certificate of Workers' Compensation Insurance; note: The State Insurance Fund provides its own version of this form, the U-26.3; or Sl-12: Certificate of Workers' Compensation Self-Insurance.
