The following documentation and administrative fee must be submitted with the Vending and Food Truck
Application to John McNerney jmcnerney@cortlandville.org for application to be considered complete.
Incomplete applications will not be accepted or reviewed.
Administrative fee of $100.
Copy of Cortland County Health Department Food Certification Permit.(753-5036)
Insurance Certificate of Liability in the amount of $1,000,000 naming the Town of Cortlandville
as an additional insured.(Email copy to jmcnerney@cortlandville.org)
Food Truck inspection completed by the Town Code Officer (Fire &Safety Inspector Desiree
Campbell 756-7490)
A written waste disposal plan for all trash,water,grease,and other materials.Town Staff will
review the description,and modifications may be required before a permit will be issued.
A copy of New York Sales Tax Certificate (www.tax.ny.us or 1-800-225-5829)
Copy of Current Vehicle Registration and Proof of Automobile Insurances required by NYS
Department of Motor Vehicles.
Description of Food Items or products offered for sale.Description of Mobile Vending unit or
photo.(For informational purposes only)
Have you ever been convicted of a felony or misdemeanor?Yes or No (Circle one)
If yes,you will be asked to provide authorization for the town to conduct a
background check.
Vending/Food Truck Fees
Daily Fee:$25/day
vendors will be invoiced at the
conclusion of dates for daily fee.
Administrative Fee:$100

Vending and Food Truck Insurance Liability Requirement:
Certificate of insurance in the amount of $1,000,000 naming the Town of Cortlandville as an additional insured
must be submitted 7 days prior to commencement of requested dates.Email completed application and proof
of insurance to jmcnerney@cortlandville.org and mail to:

Facility Rules
●Food Trucks in vending area must be approved by Town Representative.
●All vehicles must remain in the parking lot.
●Equipment is not provided by GLSC.You must bring your own bats,balls,donuts for bats,gloves,etc.
●Dogs must be kept on a leash at all times.If you do not pick up after your dog you’ll be asked to leave.
They may never be on a field of play.
●All facility equipment must stay where it is (garbage cans,bleachers,benches,goals,etc.)
●Your permit won’t be considered valid until all proper insurance paperwork is turned in
●We reserve the right to cancel or postpone any events due to inclement weather or required
maintenance
●You may not attach any of your own supplies to the facility.This includes signs,posters,balloons,
postings,etc.
●No glass containers allowed in the facility
●No metal cleats are allowed at Gutchess Lumber Sports Complex
●No sunflower seeds are allowed at Gutchess Lumber Sports Complex
Terms and Conditions
●The Gutchess Lumber Sports Complex (GLSC)has the right to retain the deposit due to any damage
incurred,failure to adhere to facility rules/regulations during rental period.
●GLSC is not responsible for lost,damaged or stolen property.
●We are not responsible for any injuries received during the rental period.
●This is a smoke-free park.Smokers will be asked to exit the premises.
●At any time,should they deem it necessary,the Gutchess Lumber Sports Complex may terminate or
nullify this agreement.Written notification will be given.
●Alcohol is prohibited.
●Rental applicants must be 21 years of age or older.
●Applications must be turned in 14 days prior to the event.

and agree to abide by the documented rules and procedures.
