DATE (CATEGORY FEMA- 1589 -NY TOH 010 023-32160-00 6/2912005 C DAMAGED FACILITY WORK COMPLETE AS OF: Roads & Culverts 7/13/2005 D % APPLICANT COUNTY Town of Harford Cortland =OCATION LATITUDE (LONGITUDE Scutt Road IN 42.46720 W 76.16840 DAMAGE DESCRIPTION AND DIMENSIONS During the declared incident period from April 2, 2005 to April 4, 2005 the Town of Harford experienced severe flooding and substantial rainfall That caused washouts and shoulder damage townwide. Along Scutt Road, a seasonal road, four sites were damaged, Site 1. A 35'L 18" CMP culvert was bent and damaged. 20'L x 5'W x 1'D ditch erosion also occured at the outlet, washing out existing riprap_ Site 2. A 100'L stretch of 16'W x 6"D road was washed out. Site 3. A 500'L x 16'W washout oc cured on the road as well as in the 5'W x 2.5'D ditch, washing out existing riprap. Site 4. A 2T long 18" CMP culvert was damaged. 6'H x 10'W x I UL riprap at the outlet was also washed away. SCOPE OF WORK Site i. Replace the 35'L 18" CMP and fill in the 29L x 5'W x 1'D ditch erosion with riprap. Site 2. Repair the 100% x 16W road with 3" aggregate sub base and 3" aggregate surface course. Site 3. Repair the 500'L x 16'W with 3" aggregate sub base and 3" aggregate surface course. Clean the 500' x 5'W x 2.5'D ditch. Site 4. Replace the 2T long 18" CMP culvert. Place large riprap at the outlet to fill the 6'H x 10'W x 101 eroded outlet. =Aitigation proposal attached to upgrade culverts. Does the scope of work change the pre -disaster conditions at the site?
