FIRE TAX DISTRIBUTION
The enclosed check represents funds collected by the New York State Insurance
Department pursuant to Sections 2118, 9104 and 9105 of the New York State
Insurance Law. The funds are for the use and benefit of the firemen protecting the
area designated on the check.
In compliance with Section 9105 of the Insurance Law, the checks are distributed as
follows:
1. Cities and Villages - To the Treasurer of the Fire Department.
2. Fire Districts - To the Treasurer of the Fire District.
3. Fire Protection Districts - To the Supervisor of the Town in which the
protection .district is located, who in his official capacity acts as the fiscal
officer of the district. He should deposit the check into the Trust and
Agency Fund and disburse checks from this fund to the fire department
or departments rendering protection to that area. When more than one
fire department renders protection, the tax monies should be divided on
a basis mutually acceptable to all companies concerned.
4. Benevolent Associations - To the Treasurer of the Association only if the
Association has been formed by statute.

12257, OF ANY CHANGE OF ADDRESS. AS INDICATED ABOVE, WE CANNOT
FORWARD CHECKS DIRECTLY TO ANY FIRE DEPARTMENT THAT IS
LOCATED WITHIN THE BOUNDARIES OF A FIRE DISTRICT OR FIRE
PROTECTION] DISTRICT.
The Insurance Department acts as a distribution center of the tax monies received
from the foreign and alien Stock insurance companies, foreign Mutual insurance
companies and Excess Line Brokers reporting fire business throughout the State of
New York. Any inquiries relative to the proper use of such monies should be
addressed to the Department of Audit and Control, Bureau of Municipal Affairs,
Alfred E. Smith State Office Building, Albany, New York 12236.
THIS CHECK SHOULD BE DEPOSITED IMMEDIATELY AND BE ENDORSED
EXACTLY AS DRAWN, INDICATING THE NAME OF THE TREASURER OR
FISCAL OFFICER. OTHERWISE, IT WILL BE RETURNED TO THE BANK FOR
PROPER ENDORSEMENT.
