Safety Committee Meeting,January 7, 1999

Meeting began at 8:10 a.m.
New Business:
Ray Manning asked if one person working at night was a safety or legal issue. Fred
Noteboom said that he will call Mark Smarr(Public Employees Safety and Health)
about this. Fred said that he will discuss this with Lovell Safety also. Judy was sure it
would not be an employment legality issue.
Fred explained to the Committee that last year we had two people working at night,
and the men were not happy about having to work nights two or three times during the
winter months. Each night the men were given a list of work to do. So, this year we
decided to try using only one person working at night, but they would not have to do
anything except patrol the roads.
Rich TenKate agreed that he is concerned for safety of the employees but that it was
nice to only have to work nights once during the winter months.
There was a discussion regarding having a cell phone available for the employee. Fred
said that a cell phone has just been purchased for this, along with another supervisor's
use during the day.
Fire and Safety Manual Outline
The Committee discussed the changes to be made to the outline of the manual. See
attached outline.
There was a discussion that the Highway/Parks Department currently has a safety
manual that needs to be updated.
It was decided to focus on one topic and then go to the next so that the manual can be
completed in phases. The first topic to focus on will be Emergency Evacuation Plans. It
was decided to look at other possible OHSA violations as the next topics for the
manual.
There was a discussion regarding involvement of the Ithaca Fire Department while the
new Town Hall is being built. This will allow for better evacuation plans, etc.

Fred asked what the "Sweeper" does. Judy explained that that person would check the
building to make sure that it was empty.
The Town needs to document that fire drills are being conducted.
There was a discussion regarding the MSDS (Material Safety Data Sheets) at Town Hall.
Judy will talk to Kristy,in Zoning, about keeping the MSDS's up-to-date. Judy has
already talked to Kristy about training employees on Right-to-Know. MSDS training is
included in Right-to-Know training.
Judy supplied a copy of the Tompkins County Mental Health Department's evacuation
plan.
It was discussed that maps where needed for egress from the buildings. It is believed
that a map of the Town Hall is on Mike Ocello's computer. Fire extinguishers need to
be checked yearly with the date when it was checked and by whom. The Committee
feels that the Town should have a training session on how to use a fire extinguisher. The
Committee asked Chuck White to make a floor plan for the Highway and for Town
Hall,which would shows how to exit the building in an emergency.
There was a discussion regarding making a procedure for the Old Town Hall. The
procedure would then be transferred to the New Town Hall. There needs to be two
separate sets of evacuation plans--one for Town Hall and one for Highway.
There was a question if we need to assign a"Sweeper" or a "Door Monitor".

Chris indicated that we need to be concisely concerned for people with allergies. Air
quality should be reviewed continuously. Examples of things to look at are: cleaning
i carpets, drapes, heaters, air conditioners, truck exhausts. This will be dealt with as a
separate issue.

Meeting began at 8:10 a.m.
The minutes from the January meeting were approved.
Christine Lehman showed the Committee maps of Town Hall--upstairs and downstairs.
These maps included sites of fire extinguishers, pull boxes, and exits. There was a
discussion regarding where to put the maps. Rich TenKate asked if they should be in
each room. Chris indicated that maybe the halls and near fire extinguishers are the only
places they are needed.
Fred reminded the Committee that we need to remember that visitors may not know
the lay out of the building or procedures the Town has established for evacuation.
Chris asked if there were any procedures as to where the maps should go. Rich
TenKate said that possibly the amount of signs to go up should be linked to the amount
of square footage a building has. Rich will look into the requirements.
Chris said that if the maps looked OK, she will have the Engineering Department
finalize them. Fred indicated that possibly the Town could have sign/maps made up
that were florescent, so they are visible in the dark or when a light flashes on them.
Judy indicated that the new Town Hall (old post office)-has maps in the basement for
evacuation.
Rich asked if the new building will be up to code, and who will be monitoring this.
David Klein, Councilman will do this, since he is very familiar with building codes.
There was a discussion regarding a map for the Highway facility.
Rich indicated that the Fire Department may be willing to help conduct fire drills. Fred
indicated that the Town needs to start having drills and documenting them. This will
be done after this Committee writes the Emergency Evacuation Plan and has informed
the employees of the Plan.
The Committee then looked at the written plan for evacuation Town Hall. There were
changes to the plan which reduced the plan to one page.

y" There were some concerned about having sweepers for the building. The job seems
very detailed. The sweepers may not be necessary.
There was a discussion regarding the need for door monitors and there function--
keeping visitor and employees from going back in the building until they are told to.
Please see the Town Hall Evacuation plan for more details and changes.
There was a discussion regarding the Highway facility plan. This will be worked on
this week. The place to meet has been changed from the trailer to the fence on the front
lawn.
Judy asked what is required to be taken out of the building if there is an evacuation.
This will have be addressed.
The question was raised if the vault(at Town Hall) is fire safe. This is not known,but
we will look into this. Another question was if the vault should be closed and locked
during a fire. Fire Department might have problems getting through the building if it is
locked.
Fred indicated that we need to address the new Town Hall building.

Highway Facility
General Evacuation Procedures
1. At the first sign of a fire (i.e. SMOKE, FLAMES, ODOR) said person(s)
shall notify all occupants of the highway facility and then call 911.
2. All employees shall leave the building by way of the nearest exit, closing
doors when possible. DO NOT LOCK DOORS.
3. All employees shall meet at the fence by the entrance gate in the front
yard. Head count shall be taken by the Administrative Assistant or the
first Supervisor on scene.
4. Employees shall not leave the assigned meeting place until directed to do
so by his or her Supervisor.
5. The above guidelines shall be followed at all times of the day or night.
Instructions to Employees
( 1. No one shall re-enter the building until cleared to do so by a Fire
Department Official and your Supervisor.
2. No rescue attempts shall be made by employees. This will be handled by
the Fire Department.
3. Fire extinguishers and/or other fire fighting equipment shall not be used
by employees unless they have received training on the use of such
equipment.
4. No one shall leave the front yard meeting place until dismissed by your
Supervisor.
5. An employee designated by a Supervisor shall allow no one by way of
foot or vehicle access beyond the front gate unless said individual is a
responding Fire Fighter or EMS Provider.

EMPLOYEE EVACUATION PROCEDURES
GENERAL EVACUATION PROCEDURES: Town Hall (Seneca Street):
At the sound of the fire alarm, all building occupants will follow these procedures:
1.) a) The Town Clerk or Deputy Town Clerk is to call 911.
b) Fire Safety Director, FSD will go to Firebox to determine location of fire
in order to inform the fire company.
2.) Leave offices immediately and close, but do not lock, office doors.
3.) First person out the front door will act as the Door Monitor. The Door
Monitor will direct all evacuating persons to the designated meeting area.
The Door Monitor will also stop all non fire fighting and non rescue
personnel from entering the building until an all clear signal is given by the
FSD.
4.) All other employees will use the nearest stairwell to evacuate the building.
Check the stairwell door for heat with the back of the hand before opening
it. If it is hot, use another stairwell as the fire may be in the stairwell with
the hot door.
5.) Once at the bottom of the stairwell, follow the exit signs to the door.
6.) Leave the building and report to the Town Clerk or Deputy at the booth in
the M & T Parking Lot.
Do NOT leave assigned meeting place until directed.
7.) STAY WELL CLEAR OF THE BUILDING

INSTRUCTIONS TO EMPLOYEES:
1.) Any person discovering fire or smoke shall immediately transmit an alarm
of fire by using the nearest pull-box signaling station.
2.) At the sounding of an alarm, all building occupants will immediately exit
the building by way of the nearest designated exit or stairwell, assisting
anyone along the way that needs help getting out.
(For post office: Elevators will not be used in the event of an emergency
evacuation signaled by the sound of the fire alarm.)
3.) All occupants of the building (employees and visitors) will meet and stay
at the booth in the M & T parking lot immediately after leaving the
building.
4.) Fire extinguishers and/or other fire-fighting equipment will not be used by
employees unless they have received training on said equipment.
5.) No rescue attempts (building re-entries) will be made by employees.
Rescues will be done by the Fire Department, if necessary.
6.) All occupants will remain in the parking lot congregating area until the all -
clear signal is given by the FSD (Fire Safety Director) as "ALL CLEAR".
A stopped alarm does not mean the building is safe to re-enter.
FSD = Director of Building/Zoning or Assistant
IF AFTER HOURS FOLLOW THESE PROCEDURES:
1.) At the sound of the alarm the chairperson is to inform the public present to
evacuate the building via the fire exit at the back of the board room and
direct them to congregate by the booth in the M & T Parking Lot.
2.) Chairperson is to or delegate someone to call 911.
3.) A staff member present shall sweep the upstairs to ensure that all have
evacuated the building. Then shall return to the board room to help
evacuate the board room.
4.) A staff member shall act as Door Monitor and as Fire Safety Director.

The meeting began at 8:07 a.m.
The February meeting minutes were approved.
Emergency Evacuation Plan
The Committee reviewed the Highway Department's evacuation plan and maps. Chris
Lehman will talk to the Engineering Department to see if they can make a map with the
exits, fire extinguisher,etc on it. There was a discussion that the Annex building needs
have a map of the exits,fire extinguishers,etc. also. There were some minor changes to
the written policy.
There was a discussion regarding evacuations near the vault. There were concerns
because the arrows on the map only show one way to go. If the vault doors are closed,
someone may become confused. The suggestion of having the arrows a different color
from the exit symbols was made.
The question of if the vault is fire proof was raised. Judy Drake will look into this.
There was a discussion regarding having the maps laminated, especially at the
Highway Department. Judy will have Joy Daley look into the cost of laminating the
maps.
There wnwac a dicr,issiori regarding condiicting fire drills. Judy will schedule a meeting
(at Town Hall) to review the instructions. Fred Noteboom asked Ray and Rich to
decide how and when the instructions and a fire drill will be conducted.
There was a discussion regarding what to take out of the buildings in case of a fire. It
was decided that the safety of the Town's employees and guests is most important.
Judy will talk to Joan Noteboom,Town Clerk, regarding what information is needed to
have the Town running within 24 hours. We will then work on getting copies of those
things off site.
Chris will have the Engineering Department make some 8.5x11 maps for the written
Fire and Safety Manual.
There was a discussion as to how often a fire drill is to be conducted. Judy felt that bi-
annually would be good.
Next Topic to Write for Manual
The Committee decided to start writing the narrative for Driver Safety.
It was decided that Hazardous Material Plan and the Confined Space Plans should be
made separate items. Because the Town of Ithaca Town Board has already approved
each of them, there will be no need to write anything on them. In the manual, we will
have them referred to.
For the next meeting, the Committee members will start investigating different training
opportunities for driver safety.
Mark suggested buying some videos as a refresher to instructor training.

The meeting began at 8:07 a.m.
The March 11, 1999, minutes were approved.
Evacuation Plan
Judy Drake informed the committee that the Town Hall evacuation maps would be
laminated. We still need to work on the Highway evacuation maps.
Judy said that Town Hall conducted an evacuation drill because the company who tests
the fire alarm system was there, so they set off the alarm. Judy indicated that most
people did not know that was the fire alarm.
After the evacuation drill, there was a meeting to discuss the procedures. There was
some concern about having employees check the building. The Committee continues to
be concerned for the safety of a person who was asked to check the building. Please see
the attached notes from that meeting.
At least two evacuation drills should be practiced a year.
Judy said that she is setting up a meeting to discuss, Right-to-Know issues, the
evacuation plan, the New Town Hall, and packing up issues.
There was a discussion regarding having fires drill at the Highway facility. Judy will
set up a meeting to go over the procedures, etc.
Chris will be leaving the Town of Ithaca so Kristi Rice will replace her on the
Committee.
Judy said that she talked to Joan Noteboom regarding what items need to be taken out
of the building. Joanie said that the Town Clerk or a Deputy would call 911. She said
that the vault does need to be closed. They will work up separate instructions for the
Town Clerk and Deputies.

The Committee decided that the Town needs to conduct fire extinguisher training and
have the fire extinguishers inspected.
Driver Safety
What do we want to include in this section? Possibly defensive driving, truck driving
school, etc. The manual should include that Driver Training is provided by the Town
for employees, every three years. There should be some information about CDL
training.
The Committee discussed having ground guides when vehicles are being backed up.
There was a discussion regarding the ways to make employees more aware of driver
safety issues. One company used Ice Cream, another issued "tickets". Another idea
was to not allow a repeat offender of driver safety to drive the vehicles. One idea was
to post the amount of days the Town has been without incident.
There was a discussion regarding investigation accidents more thoroughly. The Town
may want to adopt a standard investigation form and put them in employee's files.
It was suggested that we talk to New York State Department of Transportation about
their driver safety program.
Gail will make copies of the old safety manual and attach it to the minutes.
The next meeting will finish up the driver safety section of the manual: Reporting of
Accidents and go on to Personal Protection Equipment (PPE).

Meeting began at 8:05 a.m.
Meeting Minutes were approved.
Discussed that Chris will be leaving in June, Ray has resigned from the committee.
Judy informed the Committee that the Town Hall would like to discuss the building
evacuation plan more. They will have a meeting on Wednesday, May 12, 1999, in which
they will discuss it. Kristi felt that someone going to the firebox to see where the fire is
located is not a good idea. This plan has not been reviewed the Fire Department. Judy
will let Gail know if there are any changes to the plan. There was a concern about the
Zoning staff that being"Fire Safety Director", because they are not in the building
enough. Maybe changing the wording from Fire Safety Director to the closest person to
the box. The question was presented, "Who is responsible for this if the Fire Safety
Director is not there?" There are definite concerns about fires and evacuations for
Town Hall because of the structure and paper there.
Driver Safety
The Committee discussed the new directive that hard hats and safety vests are to be
worn at all times. Fred will talk to PESH about this. This issue will be reviewed with
daily reminders.
The Committee was given a copy of the Highway Department's safety manual,which is
very old,but a good starting point. There is a lot of good information in the manual,
but it needs to be reworded and put in the proper place.
The Committee reviewed sections of the manual and has revised the Driver Safety
section. Please see the Fire and Safety Manual for the changes.
What do we want to work on for the next meeting? Review other sections, and see
what else can be added to the driver section of the manual.

Meeting began at 8:10 a.m.
Judy Drake informed the Committee that Public Employer Risk Managers Association,
Inc. (PERMA) is the new worker's compensation company representing the Town of
Ithaca.
There was a general discussion about what types of safety issues PERMA can help the
Town with. They have videos and trainings available to the Town. There is an 800
number to call to request videos. We will need to give them a date we want the videos.
We can keep the videos for one week. There is a number to call to return the videos.
The Town of Ithaca has recently acquired a new track excavator that has no back up
alarm. This is not a standard feature for the vehicle because the driver's seat will turn
so that a driver can see both ways. Fred Noteboom asked Norm Wiley if he
recommended doing something about the excavator having no alarm: should one be
added to it or not. If an alarm were not put on the excavator, a procedure for backing
up would need to be developed, such as blowing horn twice. It was suggested the
procedures be posted in the excavator if possible.
Reviewed the Safety Manual. See the manual for revisions. The following is a
discussion regarding information for the manual.
There was a suggestion that a "vehicle list" for each vehicle be used. This would consist
of who can drive the vehicle, based on experience and training. The Town may want to
look into more formal training on equipment; as it is now,we rely on learning the
operation of equipment on using it.
There was a brief discussion as to including a section in the manual, under Reporting of
Accidents, explaining the procedure the Town will follow to investigate accidents. The
Committee will need to come up with this procedure.
Next meeting will be August 5, 1999.

Meeting began at 8:10 a.m.
The July minutes were approved.
Judy Drake informed the Committee that she just received some information from
Norm (PERMA) regarding drivers and disciplines for accidents. She will review this
and get back to the Committee on this.
The meeting was devoted to work on the Driver Safety section of the Safety Manual. It
was decided that vehicles and motor equipment did not need to be in separate
headings. This will elevate questions of what is motor equipment and what is a vehicle.
After the discussion on the Driver Safety section, the Committee decided to start the
Personal Protection Equipment(PPE) section of the Manual. Kristie asked what
equipment the Town has for response to hazardous material spills, etc. Gail read the
Towri s policy, approved by the Town Board,which indicates that the Town is only
responsible for securing the area; the Ithaca Fire Department will be responsible for the
rest. Thus, the Town does not have or need PPE for Hazardous material responses.
Next meeting will be September 2, 1999.

Safety Committee Meeting, September 2, 1999
Present: Scott Dean, Mark Cass, Kristie Rice, Fred Notebom and Judy Drake
The meeting began at 8:10 a.m.
The minutes from the last meeting were approved.
There was a discussion regarding the need for safety toed shoes. Scott Dean feels that
the employees should wear them. At this time Bolton Point requires their employees to
wear them. The employees receive a stipend for them. For the first year an employee is
at Bolton Point, they receive 2/3rd of the cost of clothing, up to$300. This includes shoe
and clothing. After that, they receive$50 per year.
The Public Works Committee has briefly discussed this issue. This Committee will
review the idea of clothing and shoe allowances making recommendations as necessary.
The Committee members will investigate the cost of safety-toed shoes and report back
at the next meeting.
The Committee discussed the fact that there is a new material on the market that is as
strong as steel, but less heavy.
There was a discussion regarding who needs to wear safety vests, hard hats, etc., and
when. This will be included in the Safety Manual.
There was a discussion regarding who is responsible for an employee refusing to wear
Personal Protective Equipment. Ultimately the employee is responsible for wearing the
PPE, although the Working Supervisors would be responsible to remind employees to
wear PPE.
The safety manual was worked on again. Please see the manual for an update.

Safety Committee Meeting, November 4, 1999

The meeting began at 8:35 a.m.
The September minutes were approved.
Judy informed the Committee that Cargill Inc., allows $80 per year for safety-toed
boots. The city of Ithaca allows $100 for any employee who does not have a uniform
provided.
She also looked into prices of purchasing safety-toed boots. At Sears, their cheapest
pair of boots is$69, Die Hard boots are$85 and Wolverine boots are$90. At Fontania's
their boots are$129 a pair.
Creig Hebdon told the Committee that in Cortland County,they contract with Lehigh
Safety Shoe to bring boots around to the Departments and employee's can purchase
boots, previously picked by the County, at a certain cost. If the cost of the boots is more
than that, the employee has to put in the rest of the money. Lehigh then sends Cortland
County an invoice.
Scott Dean explained that Cornell University allows $105 a year for safety-toed shoe
allowance. This goes up by $5 each year of their contract. They have a truck that comes
around for people to purchase boots from.
There was a discussion regarding what is considered soft-soled shoes. It was decided to
include in the Safety Manual a statement about no sneakers instead of soft-soled shoes.
Creig asked what the definition of"other people" is. Although it is not in the manual, it
is understood that it is talking about people from the Engineering, Zoning, and
Planning Departments.
There was a discussion regarding the consequences if a person was given an allowance
for safety-toed shoes and did not wear them. The person would be subject to the
diciplinary policy as a failure to follow safety procedures.

The Committee decided to refer the shoe allowance issue to the Public Works
Committee and Town Board for their approval. We will propose a resolution requiring
$100 per year for Highway Department employees and$100 every other year for other
departments.
Scott asked if the Town would replace or clean winter coveralls,which are not the Town
issued coveralls. This will be discussed more at a later date. Possibly suits could be
purchased for sewer work.
The Committee will also have the Town Board revisit the resolution from 1992
regarding prescription safety glasses. We will recommend that the Board approve $55
for all prescription eyewear,but leave the rest of the wording on the resolution the
same.
At the next meeting,we will discuss wearing safety vests and then move on to hearing
protection.

The meeting began at 8:07 a.m.
The November 4, 1999, minutes were approved.
The Committee decided to continue meeting on the first Thursday of the month in the
year 2000. A sheet with the meeting dates will be sent to members.
There was a discussion regarding which Committee to present the idea of
reimbursement for safety toed shoes.Judy Drake will write a summary of the
Committee's recommendation and send it to the Public Works Committee and the
Personnel Committee. It can then be presented to the Town Board. There was a
discussion regarding when to start this program, if approved by the respective
Committees and Town Board. We need to talk to Fred about what line item the
Highway crew's allowance would come from. No money was included in the 2000
budget for safety shoe allowances.
Kristie Rice provided the Committee with information (from the Internet) regarding
OSHA's regulations for wearing safety vests. The document(OHSA standard 1926.201)
says that red or orange garments must be worn. The Committee decided that the
manual would specify orange, since we feel that it is more visible.
Creig Hebdon researched suits for the employees who have to work on sewer
maintenance or breaks. The Town may be interested in purchasing several disposable
suits (pants and jacket) for this. Scott Dean informed the Committee that he was asking
if the Town could pay to have employee's personal coveralls cleaned. The Committee
agreed that this was not an option,but that the Town could provide disposable suits.
This will be discussed further at a later time.
The rest of the meeting was devoted to work on the safety manual.
The next meeting will be January 6, 2000. At that meeting we will make a training
schedule and review the safety manual additions.
