Update on Dangerous Intersections ghk 144/rr�� nnrE TOWN OF ITHACA Safety Committee February 3, 2006, 8:00 a.m. Public Works Facility Present: Fred Noteboom,Judy Drake,Jon Munson, Dave Boyes, Creig Hebdon, and Kristie Rice Start: 8:15 a.m. The minutes from the last meeting were approved. Creig Hebdon asked what the status of this committee was? Judy said that this committee will be considered an"internal staff committee". All of the decisions made from this committee will have to be referred to another committee such as the Personnel or Public Works to then those Committees would make recommended to the Town Board. Incident/Accident Reports There were no reports to report on. Fred Noteboom said that we might want to look into keeping track of how many incident reports the Town received in a year and where they are. This might help when scheduling repairs to roads, water lines, etc. The Committee discussed that there were a few accidents last year involving temporary employees and backing up vehicles. Should we look into not allowing temporary employees to drive Town vehicles? Fred said that he would like to have more specialized storm water management training for the Public Works Department's employees. Ergonomics Assessments Gail Kroll explained that she had Greg Shaw, who received a grant to study ergonomics,come to the office and conduct an ergonomics assessment of her station. This was very informative and has helped a lot. We talked to Greg about conducting ergonomics assessments for the equipment operators. Greg is willing to meet with one or two people (who have gotten questions 4 6p Y from other staff and who will take information back to the other staff) to talk to about ergonomics for equipment drivers. The Committee felt that Scott,Tim, and Dave would be good candidates to sit down with Greg. Fred said that we have the decimeter, and we will get it calibrates so we can use it. Building Safety Checklists The staff continues to work on these. The Public Works Facility one is almost done. The Committee has requested that we finalize them for the next meeting. There was a discussion regarding the recent fire inspection at Town Hall and that it was not clear if all the recommended changes had been made. Dangerous Intersections Fred said that the Town received a thank you letter from Will Burbank for fixing the intersection of Coy Glen Road and Elm Street Ext. Creig said that this project is on hold because they need to wait until the trees leaf out. He said that all the intersections are on the computer using the stop bar and then using the moving up method. We have not identified any other intersections to work on at this point. We may want to update the list we have to add other intersections identified as dangerous. We may want to talk to the County and the State as to their intersections that need to be addressed. Creig said that he would talk to Nicole regarding a study that she is conducting. Creig said that he did not include County to State road intersections or State to State road intersections. Others Judy Drake said that PERMA is giving money to Towns who would be willing to hold wellness fairs for other municipalities. Judy thought that maybe this committee could incorporate wellness and become a Safety and Wellness Committee. This might make more work for the committee members. We need to decide (or ask) who would like to go to the PERMA conference in May. Kristie informed Fred that he needs to request a training session regarding Meth Labs. He will do this. Judy said that the Town has purchased a CD on training sessions,so that we can plan some of our own training sessions. The next meetings agenda will include Dangerous Intersections, Training of Seasonal Staff and Finish up the Building Checklists. The Committee discussed developing a policy regarding disciplinary action(of employees) when vehicles have been in an accident. There have been many accidents in the past that goes basically unreported—lots of dings, etc. The policy should include what would happen after a person gets in an accident for the first time: Verbal warning with a statement in the personal file; second time: Written statement in file; etc. The Committee felt that if a person who is in an accident and is ticketed, there should be stiffer disciplinary action. The policy also needs to address the "chain of command"—who are employees suppose to report accidents to. Should be the same person and that person should have to document it. The definition of accident will have to be defined. The Committee felt that this policy should be basically in place before we have the defensive driver's course in October. Incidents and Accidents There were two accidents to report this time: One was for a back injury and another for a stomach injury. As the back injury was due to an employee shoveling material off the back of a one-ton truck, the Committee suggested that when patching roads, it would be better to use a box pan (that is a pan that is lower to the ground and carries material in it and is attached to the back of a truck.). The stomach injury may have been a result of only one person lifting and maybe there should have been a second to help, although the object being lifted is around 50 pounds. There was an incident that occurred involved one of the Town's vehicles, a trailer, and a truck. This incident has spurred the idea of having a policy (See above) for vehicle accidents. Aki\-1‘ 14*11 Building Inspection Checklist The checklist of the Public Works Facility has been almost completed. We would like to have a walk through and put subject matters together on the list, to make it less confusing. We will meet at 7:00 a.m. May 5th to do this walk through. Creig and Kristie were not available to report on the Town Hall's checklist. Dangerous Intersections Creig was not available to report on this. Training for Seasonal Employees One of the goals for this Committee this year is to work on and develop an orientation/training schedule for new hires. Currently,the Public Works Facility has a sheet that new hires signs stating that they have received their personal protection equipment for work. We can add to this to include other items that should be explained before new hires go out on the job. Some of the things to add should be: building tour, introduction to employees, and safety training. We also need to establish who will do the training. Miscellaneous We will be setting up a meeting with Greg Shaw to go over ergonomics with Dave Boyes and Scott Dean, who use the Town's heavy equipment a lot. Creig Hebdon will be going to the PERMA conference in May. Judy Drake said that we did not get the PERMA grant that we applied for which would have allowed us to do wellness fairs with neighboring towns, etc. The next meeting will be in June. ghk 'tit/ DISPOSITION DATE 1P4( TOWN OF ITHACA Safety Committee June 2, 2006, 8:00 a.m. Public Works Facility Present: Fred Noteboom,Creig Hebdon, Kristie Rice,Judy Drake,Jon Munson, and Dave Boyes, Start: 8:04 a.m. The minutes from the last meeting were approved. New Employee Orientation After looking at the New Employee Orientation sheets, the Committee decided to put a narrative with each section. Currently,when an Engineering intern starts,Creig Hebdon sits down with them and goes over the following: Traffic control, personnel protective equipment, cone set up, the temporary employees manual, tours the building, and watches safety videos. The Planning interns are giving a tour of the building and shown the fire evacuation schedule. We will make up a"sign off" sheet for the interns at Town Hall, similar to the Public Works sheet. We will add something to the sheets regarding the last day and getting PPE back, etc. Ergonomics Study for Public Works Employees The Committee reviewed the report from Greg Shaw and decided there would be no real benefit to having him come back. The Committee did discussion that it would be nice if handles could be put on the leaf boxes. They are very high and hard to get up to the top. It was brought to the attention of the Committee, that there are a lot of things that need to be fixed that go unnoticed. The Committee discussed that the Public Works Department might want to start having a monthly meeting with all staff to discuss these 1 things or we should make it clear that employees should report them to a Working Supervisor. Fred Noteboom said that he would be talking to the employees about the excessive use of cell phone during work hours. We all use them for work purposes,but the personal calls are getting out of hand. Creig and Kristie Rice have been working on the Town Hall's building inspection checklist. Judy Drake said that she might be able to get a grant for defensive driver's courses. She will talk to Gail to get cost estimates. Judy also said that the Town would start conducting physicals on new employees. The physicals were started because workers' comp. requires them as a baseline. We lost points in the last evaluation,because we don't do them. Creig said that we should talk to Cornell Cooperative Ext. and have a nutritionist at the Town's wellness fair. There have been several accidents to talk about and the Committee felt that we need to convey to the employees that if there is an accident,larger or small, it should be brought to a Working Supervisor's attention. Again,a chain of command should be established. Review Shoe Allowance Receipts. ghk DISPOSITION DATE _LA (.6w) l TOWN OF ITHACA Safety Committee August 4, 2006, 8:00 a.m. Public Works Facility Present: Fred Noteboom, Creig Hebdon, Judy Drake,Jon Munson, and Dave Boyes, Start: 8:07 a.m. The minutes from the June 2, 2006, meeting were approved. Incidents and Accidents There were two claims since the last meeting. One was a fall that resulted in a concussion and the other was a twisted knee. The twisted knee was red flagged from PERMA, possible due to the lateness of filing the claim. Safety Checklists The Public Works Facility's checklist is ready for someone to do a walk through. It was felt that someone like Kristen Taylor would benefit from doing an exercise like this, as she is an entry-level engineer. Possibly we would like to have Joe Hulbert go along so he knows what is needed. Creig Hebdon said that the Town Hall Facility's checklist is done except the basement. Creig will send Gail a copy of their list. Training of New/Seasonal Employees Jon Munson has been doing an orientation for the new and seasonal employees this summer. This seems to be going well. Gail and Creig will get together to work on an orientation sheet for Engineering and Planning students. Shoe Allowance The Committee reviewed the receipts received from the workers regarding how much they are paying for steel-toed boots. The Committee will recommend to the Personal Committee to raise the allowance from$100 to$120. David suggested $125. Dangerous Intersections Creig showed the Committee what has been put on the server. All the Town intersections have been put on an Access program with pictures showing the intersection. This could be helpful when trimming brush. Creig will put it on the Public Works Facility server, so it will be quicker and easier to look at. Miscellaneous Jon asked if we could get water bottles for the Bob Cat grinder. Fred thought that we already had them. Jon also felt that Dave Boyes should have another person to help with some of the loading and unloading of equipment etc. from the low boy. Agenda Items for October: Review Building Inspections. Incidents and Accidents There were no claims this month. Safety Checklists Kristie Rice will talk to Creig Hebdon to get the Town Hall safety checklist and do a walk through. The Committee would like to see Kristin Taylor and Joe Hulbert do the walk through at the Public Works Facility, and Jon Munson and Joe Hulbert do the walk through at Town Hall. The walk throughs need to be done before the December 1, 2006,meeting. Others Kristie said that the cards for the first aid course would cost$5 a person. The books (which we did not use last time) will be $20. The books are optional. Kristie said that they are getting stricter on how many people an instructor can teach at a time. The ratio is 6 to 1. Kristie will continue to work on setting up first aid training. We have not had confirmation on the wellness fair. Fred Noteboom said that the dangerous intersections work that Creig did should be incorporated into the Transportation Plan and the plan will dictate the work that gets done. The Committee needs to figure out where they stand on the issue as it complies d�' 1 with safetyand gettingthe work done. The Committee maywant to review this each year. The next meeting will be December 1, 2006, and we will set 2007 priorities and meeting dates. TOWN OF ITHACA *4( Safety Committee December 1, 2006, 8:00 a.m. Public Works Facility Present: Fred Noteboom,Judy Drake,Jon Munson, and Dave Boyes Guest: Joe Hulbert Start: 8:05 a.m. The October 6, 2006, meeting minutes were approved. The Committee discussed that the Public Works Department is currently setting up a 10-hour Competent Person training. Each person, after completion of the training, will have a certification for this type of training. Incidents and Accidents There was an incident since the last meeting. Vehicle #28 (the van) was involved in an accident going up Warren Road hill. A car slid into the van. This will be fixed,and the car owner's insurance will cover this. There were no injuries. Meeting Date for 2007 The Committee decided to keep the same dates as in 2006. Gail Kroll will distribute a list of the dates. Safety Checklists—Public Works Facility Walk Through On October 27,2006,Kristin Taylor and Joe Hulbert completed a safety inspection of the Public Works Facility. There are some issues that need to be addressed. The Committee also discussed how,when, and who will address the issues. They decided that after the walk through is complete, a list of the things that need to be addressed be put on it. A meeting with the employees will be conducted to let all employees know what is going on within the building. The Highway Superintendent will decide who takes care of the issues. The Committee feels that the safety inspection should take place in June and December each year. Because it is a long list(took over two hours), the Committee thought that the sections of the building could broken up and assigned to different people (always 44; 41laving at least two people in a section), i.e.: 2 guys work on the office/garage and 2 guys do the woodshop and outbuildings. The Committee discussed that the lighting in the garage area is getting dimmer. This is something that needs to be looked at. The lighting at Town Hall is also an issue that needs to be addressed. Fred said that he would be interested in moving to having PPE signed out and not get anything unless the old,worn out stuff is brought back. 2007 Priorities The Safety Committee decided that the following should be goals and priorities in 2007. + Review Safety Manual. a Review Inspection Check List Program. a Set up Safety Incentive Program. + Set up Tailgate Safety Training Program.
