STATE ENVIRONMENTAL QUALITY REVIEW RESOLUTION -
DECLARATION OF LEAD AGENCY, DORMADY - HILLCREST SUBDIVISION,
HILLCREST ROAD/FOREST ACRES DRIVE, TAX PARCEL NO.44.4-4.2
WHEREAS, an Application was submitted for Subdivision review by Ed Dormady on behalf
of Helen Krizek Dormady, Owner, for the proposed subdivision of the existi%25.20-acre
parcel into two parcels: Parcel A, measuring 6.943 acres and vacant; and Parcel B,18:140 acres
and vacant, Lansing, New York, also known as Tax Parcel Number 44.-1`-4,.2 imthe Residential
Low Density (R1) Zoning District; and .
WHEREAS, this is a proposed action reviewed under the Ton'-( f�aan�sMg Subdivision Rules
and Regulations, for which the completed application was received, januaKy 28, 2020; and

WHEREAS, 6 NYCRR § 617 of the State Environmental Qu 11 ty Review Act ("SEQRA")
requires that a Lead Agency be established for conducting environmental review of projects in
accordance with local and state environmental law aOhe Leadlf gency shall be that local

environmental review for the action of Subdivision review for Town of Lansing Tax Parcel
Number 44: -1-4.2 for Helen Krizek Dormady, Owners, Ed Dormady, Applicant.
