Based on the application information provided, and in accordance with the provisions of the:County Sanitary Code,you are hereby
granted permission to construct an onsite wastewater treatment system at the above location. Significant changes could invalidate the
permit. This permit is valid for two years from the date of issue. Permit may be renewed for a period of two more years, or
transferred to another person,if the application is made prior to expiration.

Conditions of Approval:
1. THAT the facilities shall be fully constructed and completed in compliance with the plans and specifications as
approved.
2. THAT the construction of the facilities shall be under the supervision of a person or firm qualified to practice
professional engineering in the State of New York under the Education Law of the State of New York,and the
professional engineer shall certify to the Department and to the permittee that the constructed facilities have fully
been completed in accordance with the approved engineering reports,plans and specifications.
3. THAT the Tompkins County Health Department be notified before the sewage system.has been covered.Health
Department inspection also may be required.
4. THAT a New York State licensed engineer or architect inspect the sewage system to insure that it has been
installed in accordance with the approved plans,provide inspection certification(in writing)to the
Tompkins County Health Department within 30 days upon completion of inspection,and provide location
measurements to all sewage system components such as the septic tank,pump chamber,distribution box(s),
etc.in relation to the home or other landmark.
