HomeMy WebLinkAbout02-05-C7.1.1STREET WORK PERMIT GENERAL NOTES 1.Contractor shall obtain Street Work Permit from the City of Ithaca prior to performing any operations within the public street rights-of-way. 2. Notify Ray Benjamin at the City of Ithaca DPW Streets and Facilities Division, (607) 272-1718, three working days prior to working in the public Right-of-Way. 3. Notify DIG Safely New York three working days prior to performing any earthwork activities at 1-800-962-7962, for a utility stake-out. 4. The Roadway shall be kept clean of mud and debris at all times. 5. Roadside drainage shall be maintained at all times. 6. Materials, equipment and vehicles shall not be stored or parked within the public Right-of-Way. 7. No night work will be allowed unless prior approval is given by the City DPW. Additional maintenance and protection of traffic may be required including lighting. 8. All maintenance and protection of traffic (M&PT) shall comply with the 2009 edition of the National Manual on Uniform Traffic Control Devices (MUTCD) for streets and highways and the New York State Supplement, and shall be in accordance with the City of Ithaca Work Permit. MAINTENANCE AND PROTECTION OF TRAFFIC (M&PT) NOTES (APPLIBALE TO ALL M&PT PLANS) General Notes: 1. All maintenance and protection of traffic (M&PT) shall comply with: x 2009 edition of the National Manual on Uniform Traffic Control Devices (MUTCD) and New York State Department of Transportation (NYSDOT) Supplement x February 2009 NYSDOT Work Zone Traffic Control manual. x NYSDOT Standard Sheets (Group 619). 2. Contractor shall obtain Street Work Permit from the City of Ithaca prior to performing any operations within the public street rights-of-way. 3. Contractor shall obtain approval from City of Ithaca in advance of closure of all sidewalks, shoulders, bike lanes and/or driving lanes. 4. Obtain approval from City of Ithaca for all changes in scope to the M&PT plans prior to implementation of such changes. 5. The contractor shall provide the City, in writing, with the names and telephone numbers of staff who are authorized to secure labor, materials, and equipments for emergency repairs outside normal working hours. Activity Area x The contractor shall maintain a minimum 500’ detween construction operations on alternate sides of the street, unless otherwise approved by the City of Ithaca. x When two or more areas are adjacent, overlap, or are in close proximity, the contractor shall ensure there are no conflicting signs and that continuity is maintained throughout all work areas. Signs: 1. The locations of the signs shown on the work zone traffic control plans and the details may be adjusted based on sight distance and other considerations. The final locations of signs are subject to approval of the City of Ithaca. 2. Any existing signs, including overhead signs, which conflict with the temporary traffic control sign layout shall be covered, removed, stored or reset, as approved by the City. All existing signs shall be restored to their original condition and/or location unless otherwise authorized by the City. 3. Signs at or near intersections shall be placed so that they do not obstruct a motorist’s line of sight. 4. The dimensions of work zone traffic control signs are described in the MUTCD. Any changes to the dimensions shall be approved by the City. 5. Provide sign supports per NYSDOT Standard Sheet 904-04, “Portable Temporary Wooden Sign Supports,” or as approved by City of Ithaca. Channelized Devices: 1. Where possible all channelizing and guiding devices are to be placed so as to provide a minimum of 2 feet lateral clearance to the travel way. Public Access: 1. Property owners whose driveways will be made inaccessible shall be notified by the contractor at least 24 hours prior to restricting use of the driveway. For multiple access properties, at least one driveway shall be open at all times. Access shall be restored to all driveways as soon as possible. 2. Suitable ramps shall be installed to maintain smooth transitions from driveways to and from the work area. Lane Closures: 1. The City may require that all lanes be re-opened at any time if the route is needed for emergency purposes. This could include incidents at locations outside the contract limits. Lane Widths: 1. Unless authorized by the City, the minimum lane widths for work zone travel shall be 10 feet. Barrier Vehicles: 1. Barrier vehicles shall be required as per NYSDOT Standard Sheet 619-11. 2. No work activity, equipment, vehicles and/or materials shall be located between the barrier vehicle and the active work area (roll ahead distance).
