No. 12808549 Delta Project No.: 2019.003.332 Dear Mr. Houseknecht: Enclosed, please find the Asbestos Bulk Sample Report Form, the associated Laboratory Analytical Result Sheets,
Material Photos, and the Sample Location Drawing for the bulk sampling performed by Delta Certified Inspector Thomas Ferro. The sampling was performed on August 8th, 2020 and addressed
suspect materials with the potential to be impacted by the upcoming Olin Hall Rooms B40 and B40B Hormozi Lab Renovation Project. Based on a review of the Design Development Drawing
Set dated August 4th, 2020, previous Delta sampling efforts, and a visual inspection of the renovation area, a total of eight (8) bulk samples were collected representing four (4) suspect
homogenous materials. Four (4) of the samples collected were “Non-Friable Organically Bound” (NOB) representing two (2) homogenous materials. The remaining four (4) samples were friable,
“non-NOB” materials, representing two (2) homogenous materials. Results for the countertop seam epoxy were reported as “Asbestos Containing”. Results for all other samples were reported
as “No Asbestos Detected”. In addition to the samples collected, other suspect homogenous materials were observed to be present but not sampled as they were addressed through previous
Delta sampling efforts. These materials included: • 12” x 12” White with Gray Mottled Floor Tile and its associated Mastic – Both No Asbestos Detected • 2’ x 2’ Smooth Ceiling Tile
– No Asbestos Detected • Epoxy Flooring – No Asbestos Detected • Sheetrock – No Asbestos Detected • Joint Compound – No Asbestos Detected • Trespa Labtop/Countertop – Non-Suspect •
Mastic from 4” Non–suspect vinyl black cove base – No Asbestos Detected Based on the above, the only asbestos containing material to be impacted as a part of the Olin Hall Rooms B40
and B40B Hormozi Lab Renovation Project is the Seam Epoxy present on the 4 seams of the Room B40 Countertop. The total quantity of the ACM Seam Epoxy is less than 1 square foot (based
on 23 linear feet at an average 1/8” width for the 4 seams where present). Bulk sample analysis was performed by AmeriSci New York, Inc., an independent laboratory approved / accredited
by the NYS Department of Health (ELAP), the American Industrial Hygiene Association (AIHA), and the National Voluntary Laboratory Accreditation Program (NVLAP). Analysis of all Non-Friable
Organically Bound (NOB) materials was initially performed by Polarized Light Microscopy (PLM) following the NYS DOH “We are a seamless extension of our clients’ organizations” DELTA
ENGINEERS, ARCHITECTS, & LAND SURVEYORS, DPC Page 2 of 9 AN ISO 9001:2015 CERTIFIED COMPANY ELAP 198.6 Methodologies. If the PLM results were reported as “non-asbestos”, the sample
was then analyzed by Transmission Electron Microscopy (TEM) following the NYS DOH ELAP 198.4 Methodology. Analysis of all Non-NOB materials were performed by Polarized Light Microscopy
(PLM) following the NYS DOH ELAP 198.1 Methodology. “Positive Stop” sample analysis protocol was utilized for a given homogenous material set with multiple samples and based on this;
7 of the 8 samples collected were analyzed. Please reference the Asbestos Bulk Sample Report Form for sample particulars and details. I have also attached Delta Company, Personnel
