Health Club Reimbursement Po /icy
Revision - March 2001
The Wellness Committee has met over the past several months to consider requests for
reimbursement for fitness activity participation.
After careful consideration and review, the Wellness Committee has decided to:
Reimburse employees for fitness facility membership OR participation in a group fitness class*
that focuses primarily on aerobic activity. Employees will be reimbursed for pre -paid,
completed, and continuous 6 or 12 months of membership or participation.
Amount of reimbursement: $75 for 6 months; $150 for 12 months. ( *Group fitness class
reimbursements may be submitted beginning March 31,2001 for the previous 6 months.)
To qualify for reimbursement:
1. The facility or class must have a staff member on site, who is CPR certified. The facility or
class must have a medical emergency plan in place.
2. Employees are required to have a completed health risk assessment, (also known as PAR -Q)
provided by the facility or class, on file with the facility.
3. Employees must provide a paid receipt or statement from the facility or class that indicates
that the employee has paid in full for the 6 or 12 months of completed, continuous, membership
or participation. (A contract stating that the payment will be paid monthly is not acceptable.)
4. Membership or participation must be in effect only during the time of employment with the
County.
5. Retirees are not eligible for reimbursement.
6. Fill out the revised form and with your proof of membership or participation, send via inter-
office mail to: Theresa Lyczko, Health Promotion Program, Health Department.

cardiovascular disease and other chronic conditions through regular physical activity, sound
nutritional practices and avoidance of tobacco products.
