reference are set forth in full on the FLTG website at www.fltg.com. By signing below,Customer agrees that it has reviewed this SC,STBC and any applicable
SLA and agrees to be bound by the terms. Customer hereby authorizes FLTG to act as agent for all matters relating to the provision of services listed in this

configurations. FLTG can provide services at an additional cost. Any additional equipment will be quoted separately.
