This law firm serves as outside general counsel to Carlos Leonel Cruz in connection with
certain issues arising out of the traffic accident involving Mr. Cruz that occurred on or about
January 26, 2015 (the “Accident”). We request that you identify and provide copies all reports in
the possession or control of the Village of Cayuga Heights New York regarding the Accident.
All questions or communications regarding this matter should be directed to the
undersigned. If the documents cannot be sent to me by email, we authorize Joleen Multari to
pick them up from you on our behalf.
